• Certified transcripts showing all previous academic work, including post-secondary education.
• Medical information forms, which include:
• A Physician’s Evaluation Form completed by a doctor.
• A Medical History Form filled out by a doctor, confirming the applicant is fit both mentally and physically for college studies.
• Proof of MMR (measles, mumps, and rubella) immunization.
• Proof of immunization for meningitis and hepatitis B, or a signed waiver stating the applicant has chosen not to be vaccinated.
• Proof of medical insurance that meets the university's requirements.
• Certification of finances.
• Proof of English language proficiency.
• Standardized test scores (ACT or SAT).
