Step One: Additional Documents Required

Applicants must submit certified transcripts showing all previous academic work, including post-secondary education.

Applicants must complete and submit the following medical information:

• A Physician’s Evaluation Form and a Medical History Form completed by a doctor, stating the applicant’s physical and mental fitness for college-level studies.
• Documentation of MMR (measles, mumps, and rubella) immunization.
• Proof of immunization for meningitis and hepatitis B, or a signed waiver indicating the applicant’s informed choice not to be vaccinated.

Applicants must provide proof of medical insurance that meets the University’s policy requirements.

Certification of finances is also required.

Proof of English language proficiency must be submitted.

A standardized test score (ACT or SAT) is necessary.
